Patient Confidentiality
New health privacy laws (HIPAA) in the USA are coming into enforcement.
Some of the new rules will govern how patient data is sent over the wire.  It will have to be signed or encrypted or both.

S/Mime is becoming the preferred type, mainly because MS Exchange Server/OE/Outlook has it so well integrated.  MS Exchange is expensive and difficult to setup; both these issues are feasible for large institutions e.g. hospitals where they have a large IT department, but in the small doctor's office environment for example, they will need a program that will be seamless or transparent and easy to set up with little technical knowledge required.  Signing everything that goes out and decrypting everything that comes in before it hits the desktop mail client would prove difficult for most offices in terms of setting up a new dedicated server running MS Exchange.
